For a five gallon batch:

2.5 Gallons peat moss
2.5 gallons vermiculite or perlite* (gives potasium)
9 tablespoons dolmitic lime (buy the granular, not the pellets)
5 tablespoons gypsum (gives calcium)
2.5 tablespoons super phosphate (gives phosphorus)
1 tablespoon calcium nitrate (give nitrogen)
